Companies are aren't going to just give you money. They don't have much incentive to do that. Most people take their 3% cost of living increase (plus bonus and a few % if you are lucky).
Most of my contemporaries agree. You have to switch jobs.

If you don't ask for it, you aren't going to get it. HR depts have more flexibility on hiring than raises. So coming in, make sure you get the salary/bonus/vacation/etc you want right off the bat.
